40 pitch first inning is EXACTLY what was needed. The 3 runs are just an added bonus.  They should be into the bullpen by the 4th inning, if not sooner.

If there are less than two outs and first is occupied pre-pitch (so a steal would not negate this), a runner may not advance to first on a dropped-third. With two outs, this is ignored and hitters can advance, regardless of who occupies first.
