No acute injury event and a back injury is pretty scary. Makes me think of Kris Bryant, which is probably unfair.

There are many long, thin muscles in the lower back and shoulder area that take a long time to heal. If it were a structural thing like Bryant, that would be really bad. Hopefully, it's not something like that, but saying not one event caused it is kind of too much and too little information at the same time.
