This is terrifying. I can't remember a fan ever dying from a foul ball. I'm surprised there aren't more fan injuries, quite honestly.

I'm surprised there aren't more fans hurt either, I remember at least on person dying by a puck at a NHL game in the last 10-15 years (think it was a little girl).

That was here in Columbus (she didn't die at the game, but later due to the injury sustained from it) and its why every arena now has netting in the end zones

I was thinking yesterday how dangerous it is for fans behind the third base dugout when a hitter like Lester is facing super high velocity from Cashner.

Didn't the Royals 1B coach die a few years back after getting nailed by a liner? Or was that a minor league coach?

some minor league coach got hit in the back of the head by a line drive. It's why they have to wear batter's helmets now

Mike Coolbaugh (had to google the name) was a Rockies minor league 1B coach.
