Didn't have control and ball was on the ground a bit after it slid through the receiver's hands!

I havn't seen a replay yet, but why risk it on a 4 yrd play that doesn't get you first down, and in fact still leaves it 3 and a long 9?

No, it only goes to the booth if the play starts after 2 minute warning. If its before, it doesn't matter.

It's actually happened in 3 different games I've seen in the past 8 days, which is weird since I'd never seen it before.

edit: I mean a challengee on a play that started before the 2 minute warning but ended inside 2 minutes

A challenge doesn't count as a 2-minute warning, sadly. But I'm sure the refs don't mind being out there another few minutes.
