I don't care what Kyle says, the Ian Wheeler storyline was heartbreaking. Yes, Hard Knocks probably pumped him up and he was never making the team, but you don't like to see anyone's season end that way -

Paul Skenes has a 2.9 fWAR, which would make him top 20 in the majors (top 10 in the NL), only he doesn't have enough innings to qualify for the leaderboard. He's pitching at just under a 6 fWAR pace over a 200 IP full season

I know this wasn't a "has never happened" event, but when the Marlins hitter was intentionally walked but never actually touched first base before being pinch run for, Pat and Ryan both said they'd never seen that happen before
